Please submit a hard copy of this duly filled form  and Class time table signed and stamped by your Head of Department with a passport size photograph at the office of Director PICS by due date.
Only the duly filled hard copies submitted at the office of PICS will be considered for admission.
 

and also HSK-2 applicants are required to submit either an HSK-1 certificate or a screenshot of their HSK-1 result card.
